Phone-friendly caregiver timesheet

A one-page fillable PDF timesheet that home-care staff can complete and sign on a phone.

The problem

Printed timesheets get lost and miscounted. This one adds up the hours itself and is signed on the phone.

What I built

• Seven day rows: hours fill themselves from time in and time out, overnight shifts included; the weekly total adds itself up.
• Service tick boxes, notes, and signature boxes for the employee and the client.
• Fields sized for fingers (22 pt boxes, 11 pt text).

How it was tested

• 8 of 8 form tests: field types and formats, calculations, signatures, field sizes, no overlaps, entries kept after save and reopen.
• The hours formula checked on 6 cases, including a shift past midnight.
